* For claims over 50 per month, you choose either the fixed overage fee OR a 9% contingency of recovered amount. ⚠️ Medicare legally keeps unpaid claims after 12 months — we recover before that deadline.
Koista acts as a data entry bridge. We reconcile, format, and resubmit claims based strictly on the clinical notes, system data, and written instructions provided by your practice. While we ensure technical compliance with Medicare's billing rules (such as correct fields and PRODA formatting), the clinical justification, accuracy of source data, medical necessity, and service verification for each claim remain the sole responsibility of the treating provider and the practice itself. Koista does not provide clinical advice or Medicare compliance opinions.
